Apply for Certification
Once you complete your training and are ready to apply for AMT certification, download a copy of the AMT Medical Assistant Candidate Handbook, which provides a detailed look at the certification process (Download).
When you are ready to submit an application, simply download the document below, complete it and send it to AMT. Include all pertinent documents as well as the application fee. Please note that AMT cannot begin processing until the application is complete and all documents have been received. AMT DOES NOT ACCEPT FAXED OR EMAILED APPLICATIONS. Please note that the application fee is NON-REFUNDABLE and that an annual fee of $50 is required to maintain certification.

Testing Accommodations
Attached is the AMT policy regarding candidate requirements for submitting requests under the Americans with Disabilities Act (ADA). All requests for accommodations should be made at the time of initial application to AMT rather than after a candidate has been authorized to take an examination. (Download)

Note to California Phlebotomists
California REQUIRES that all Phlebotomy Technicians or Phlebotomists who work in California hold a California Phlebotomy License. One of the requirements of licensure is that all Phlebotomists must take and pass a Phlebotomy certification examination from an approved certifying organization. AMT is approved as a certifying organization by the State of California. Please note that the application above is to take the approved AMT Phlebotomy examination, and DOES NOT double as the application for the State of California. A separate application must be submitted to the State (only the application, no exam would be required if taking the AMT exam).
