Employee Verification

AMT can verify an individual's certification and membership status to employers, potential employers, and some state licensure agencies at no charge. To receive this verification, simply complete the form below. The information you will need to provide:

• the member's name, (and if the name has changed recently, the previous name)
• the member's address
• the member's social security number
• the member's AMT ID number if available

***Employers, potential employers, or those involved with state licensure should also provide information regarding their company's name, address, and reason for the inquiry.

Once the request is received, AMT will respond with the following information within 1 business day:

• Confirmation of whether or when an individual received his/her AMT certification
• Confirmation of which AMT certification the individual was awarded
• Information regarding whether the individual has a current certification
