CCP Forms

Use the following forms to document your activities related to the Certification Continuation Program (CCP):

Compliance Evaluation Worksheet & Attestation Form (Download) - general form to be completed every three years. AMT will let you know when the form is due. Please keep backup documentation for all of your activities through the use of forms below. These forms DO NOT need to be submitted to AMT unless you are requested to do at the time of an audit.

• Continuing Education Credit Request Form (Download)- to be used to document professional education and formal education activities. You may opt to have AMT keep track of your CE credits through the AMTIE CE Recording Program. For more information on this program (Click Here)

• Employment Verification Form (Download) - to be used to document employment in your particular field

• Professional Activity Credit Request Form (Download)- to be used to document authorship of written works, instructional presentations, organizational participation and other professional development activities.
