Certification Application

AMT certification applications is downloadable from this page (in PDF format). Adobe Acrobat Reader Software is needed to view and print PDF files.  If you do not already have the application, please click the link below to obtain your FREE copy. Applications are listed two ways:  PDF: where you have to print off the application and hand write all information or PDF Fill In: where you can type the information into the fields, print it off, sign it and mail it to AMT. After you print an application it may be completed and sent to AMT as an ACCEPTABLE ORIGINAL. ATTENTION:  International applicants have different Medical Technologists and Medial Assistant Applications.  Please make sure you choose the correct application. AMT does not test in Saudi Arabia or United Arab Emirates anymore.  To find the nearest international testing location, please visit the Pearson Vue website at www.pearsonvue.com.

NOTICE TO INTERNATIONAL APPLICANTS: You must get your Foreign Educational Credentials evaluated PRIOR to examination.  There are a number of companies that perform evaluations of foreign educational credentials.  One source of information on contacting such organizations is to go to the website of the National Association of Credential Evaluation Services (NACES) at www.naces.org, and review the list of their current members at www.naces.org/members.htm If you have any questions on this requirement, please email AMT.

LOCATE A TESTING CENTER FOR INTERNATIONAL TESTING To find a testing location, please visit the Pearson Vue website, www.pearsonvue.com.  Testing is now available in the Philippines. To find the testing center, please log onto the above mentioned website.

Medical Technologist (MT) Application - All applicable information, signatures and notarized documents must be completed in order to have your application processed.  Once completed please send to AMT along with your payment. Examinee Agreement -This form must be included with your application or processing will be delayed.
