The following four documents are very important for California phlebotomists. 


 Phlebotomy Licensure Law Instruction Sheet - This sheet spells out the requirements for licensure as a Phlebotomy Technician in California. It also includes instructions line by line for the license application. (45 KB)

 Phlebotomy Program Overview Sheet - Gives a brief overview of the requirements and documents needed for your California license. (41 KB)

 Phlebotomy Technician Certification Application - This is the actual certification application. This form can be filled in from your computer, printed off and submitted with all paperwork needed for licensure. (181 KB)

 Phlebotomy Puncture Performance Sheet - This form must accompany your application for licensure. (33 KB)
