Certification Application

AMT certification application is downloadable from this page (in PDF format). Adobe Acrobat Reader Software is needed to view and print PDF files.  If you do not already have the application, please click the link below to obtain your FREE copy. Applications are listed two ways:  PDF: where you have to print off the application and hand write all information or PDF Fill In: where you can type the information into the fields, print it off, sign it and mail it to AMT. After you print an application it may be completed and sent to AMT as an ACCEPTABLE ORIGINAL.

Certified Medical Administrative Specialist (CMAS) Application.pdf - All applicable information, signatures and notarized documents must be completed in order to have your application processed. Once completed please send to AMT along with your payment. AMT WILL NOT ACCEPT FAXED OR E-MAILED APPLICATIONS. Examinee Agreement (77 KB)-This form must be included with your application or processing will be delayed.
