Certification Application

AMT certification application is downloadable from this page (in PDF format). Adobe Acrobat Reader Software is needed to view and print PDF files.  If you do not already have the application, please click the link below to obtain your FREE copy. Applications are listed two ways:  PDF: where you have to print off the application and hand write all information or PDF Fill In: where you can type the information into the fields, print it off, sign it and mail it to AMT. After you print an application it may be completed and sent to AMT as an ACCEPTABLE ORIGINAL.

Registered Phlebotomy Technician (RPT) Application.pdf - Please print the application, fill in your information completely, sign the back of the application, include payment and send it to AMT.  There are two other documents that need to be sent with this application - reference sheet and venipuncture sheet (see below documents).

Examinee Agreement (77 KB)-This form must be included with your application or processing will be delayed.
