Intent of Scholarship
In an effort to address a persistent and deepening national need, Dade Behring is providing financial assistance and professional encouragement to young clinical laboratory professionals already enrolled in a structured (NAACLS accredited CLT/MLT) course of study. The hope is to add to the corps of highly skilled professionals that constitute the backbone of the life-saving work performed in clinical laboratories all over the nation every day.
Eligibility Requirements
1. The applicant is enrolled in a NAACLS -accredited Clinical Laboratory Technician/Medical Laboratory Technician program.
Note: Students in Clinical Laboratory Scientist/Medical Technologist programs are NOT eligible for this scholarship.
2. The applicant has a 2.5 or better GPA.
3. The applicant will be completing his or her final year of study by August 31, 2006.
Terms of Scholarship
1. Fifty scholarships of $1000 each will be awarded.
2. The scholarship award is intended to be used for tuition, fees or other requirements of the academic institution.
3. The scholarship will be awarded September 2005.
Application Procedure
1. Please complete the entire scholarship application form. If information is not applicable, please indicate inapplicability with an "N/A" in any unanswered section.
2. Supportive documentation required:
A. A copy of the student's academic record MUST be included in this application. This must include all courses taken at any post-secondary institution that have any bearing on the current course of study. The transcript(s) may be a photocopy or internally generated document. An official transcript is not required.
B. Two letters of reference MUST be included with this application. The applicant must obtain two (2) written recommendations, one from a faculty member in his/her program of study and the second from an employer, community leader, agency personnel or appropriate school official (other than a CLT/MLT instructor).
C. A separate statement explaining why the applicant chose this field MUST be attached to this application.
D. The application packet will be considered complete only if it contains all of the following:
· Completed scholarship application form
· Student’s academic record from all post-secondary institutions
· Letter of recommendation from a program faculty member
· Second letter of recommendation
· Student statement explaining why the applicant chose the CLS/MLT profession
NOTE: INCOMPLETE APPLICATIONS WILL NOT BE PROCESSED. APPLICATIONS WILL BE CONSIDERED INCOMPLETE IF THE SCHOLARSHIP APPLICATION FORM IS NOT FULLY COMPLETED. THE APPLICATION MUST INCLUDE THE STUDENT'S ACADEMIC RECORD, LETTERS OF REFERENCE, STUDENT STATEMENT, AND SIX COPIES OF THE PACKET IN A SINGLE ENVELOPE.
